I have tried all speeds and even the ID reads give different results every time (and none of the ID read attempts match 208d as the data sheet specifies).

This is a known-good EPROM out of a working device.

I have successfully used this reader to read other EPROMS in the past (but not of this type) without any issues.

 Thoughts?

Check your pin for good contact between chip-adapter-programmer.

You should be able to get a chip ID before doing something else.

Yep, I already verified that. The adapter is brand new, too (the programmer itself is about a year old, and works fine with other devices). I see another post further down where someone else is having problems with the Chip ID on the v4 box (different chip) and there was a post that there could be a new bug in the firmware/software?
