I am having a problem with my programmer, PCB3. I am able to read and verify blank 27C512 eproms, but any time I try to write data I get the error shown below.

It will not verify and when I read the chip again, it's just a mess or unintelligible garble that bears no resemblance to the original bin file. What am I doing wrong? I have followed all the instruction, and believe I have the jumpers set up correctly along with correct dip switch positions. I am providing power from a 12v wall wart.
